This is an update from Sound Pediatrics. Medical Records are now being stored at Peninsula Community Health Services (PCHS) who has agreed to be our Records Custodian. Any future requests for medical records should now be directed to PCHS. You can find a PCHS Release of Information Form for ingoing and outgoing records below. The requests need to be faxed or mailed to PCHS.
